Practice Policies


Your Location

You must be in Washington state during your appointment. Please be sure you have good internet access and are in a private location where you feel comfortable disclosing personal information. Your provider is also located in-state in the privacy of a remote office.


Cancellation / No-Show

If cancellation occurs at least 24 hours prior to our session, there is no charge. If the cancellation occurs less than 24 hours prior to the session, you will be charged $200. This fee is not covered by insurance and cannot be submitted for insurance reimbursement.


Tardiness

If you arrive more than 10 minutes late for an appointment without notification, you may need to reschedule to ensure that adequate time and attention can be given to your care. Please note that you will still be responsible for the payment of the missed session.


Administrative Requests

If you need documentation, please discuss it with your provider. This service is available to established patients and includes family medical leave of absence (FMLA), gender-affirming procedure letters, and emotional support animal (ESA) letters.


Emergencies

In the event of an emergency, please call 911 and go to the nearest emergency department. In the event of a mental health crisis, call or text 988 or contact a crisis counselor at crisistextline.org.


Refills

During your appointment, you’ll receive enough refills to last until your next visit. If you need a refill before then, please contact your provider through the Patient Portal. Make sure to schedule an appointment and include the following details: your pharmacy, medication name and dose, and your next appointment date. If your medication needs adjusting, an appointment will be necessary.

Due to the significant administrative burden, we cannot respond to pharmacy-initiated refill requests. Please opt out of this service if it is offered by your pharmacy.

Controlled substances will not be refilled outside of office visits. Please refer to your Controlled Substance Agreement for more information.


Controlled Substances

If prescribed controlled substances during treatment, patients will need to complete an annual in-person physical exam and periodically monitor vital signs, such as blood pressure and heart rate. Additionally, you will review and sign a Controlled Substance Agreement prior to starting these medications.
